OK, got the promised "further word" from Prudential, and they claim that at the time of enrollment we needed to furnish proof of insurability and didn't (which the guy on the phone said was a questionnaire that needed to be mailed in, not a full physical exam). Now, I was being all hands-off on the insurance enrollment business, but I do recall H mailing something to do with that, because having outgoing mail is not common at our house and so there was discussion about how best to do it. Unfortunately, I don't recall getting confirmation back from Prudential, and that is what they want, but I have 180 days to find such confirmation and appeal, and even with my office in complete disarray, in 180 days I will find it if it's there. I suspect that even if it really should be, it won't be, considering they had the wrong address for us when it was time to send out the condolence letter, etc. But, whatever, I have to look.
This shifts my priorities as far as what to be doing with my time significantly. Kind of good, because I was dithering on what to do first/next, but also annoying because it's definitely not what I would have chosen to prioritize. But it would have had to have been done eventually, anyway, so, whatever.
